How to get a death certificate in Perth

The Death Certificate & Who To Notify

Understand the essential process of death registration and obtaining a death certificate in Western Australia. Our guide clarifies the steps, required information, and how Mayfair Funerals Perth assists families through this crucial process.

Death Certificate And Death Registration

Your funeral director will submit the notification of your loved one’s passing to the Registry of Births Deaths and Marriages within 14 days after their direct cremation or funeral service.

This process requires your funeral director to submit the medical cause of death provided by your loved one’s treating Medical Practitioner or documentation provided by the Coroner in addition to information about your loved one that you will provide your funeral director. Once the registration is complete, the Registry of Births, Deaths and Marriages can issue a death certificate, which is often required for legal and financial matters.

If you have requested a physical copy of the death certificate your funeral director will reach out to you on its arrival and make a time for you to attend our office to collect it. Please note that it may take up to two weeks for its arrival to our office.

If you have opted for a direct cremation for your loved one you will have been provided with a link to request a physical copy of the death certificate should you wish. Alternatively, if you would like to receive the death certificate sooner, you may choose to go into your local courthouse to purchase this once they have completed their processing.

When applying for a death certificate via the link or at the courthouse you must provide proof of identity, details of the deceased person (full name, date of birth, date of death), and any supporting legal documents required.

Please note that if you are organising a direct cremation and are not an immediate family member such as a Parent, Husband, Wife, Child, legal representative, or an executor of the loved one’s estate and you require a copy of the death certificate, we recommend that you order a physical copy from your funeral director at the time of arrangement.
